Managing Groups and Fields for Outcome Measures
Outcome Measures allow you to record and track standardised clinical scores, such as GAD-7 or PHQ-9, over time. Once configured, Outcome Measures can be linked to assessment forms, allowing scores to be calculated automatically and reported on without the need for manual calculations.
Before you can use Outcome Measures in an assessment, you must first create them under Fields.
Outcome Measure Groups
Outcome Measure groups help you organise related Outcome Measures. For example, you may wish to create separate groups for Mental Health, Musculoskeletal, or Paediatrics, for example.
Create a Group
To create a new group:
- Go to Main Menu → Fields → Outcome Measures.
- Select Add Group.

- Enter a name for the group.

- Save your changes.
Rename a Group
To rename an existing group:
- Open Main Menu → Fields → Outcome Measures.
- Select the group you wish to rename from the Group dropdown.
- Click Rename Group

- Edit the group name.
- Save your changes.
Remove a Group
If a group is no longer required, it can be removed.
- Open Main Menu → Fields → Outcome Measures.
- Select the group you wish to remove from the Group dropdown.
- Click Remove Group

- Confirm in the modal
Note: A group should only be removed if it is no longer being used by any Outcome Measures. If Outcome Measures are assigned to the group, consider moving them to another group first.
Outcome Measure Fields
Once you have created a group, you can add one or more Outcome Measures to it.
Create an Outcome Measure
To create a new Outcome Measure field:
- Go to Main Menu → Fields → Outcome Measures.
- Select the +Add button

- Complete the required fields for:
Name — Enter a clear, descriptive name for the Outcome Measure, for example "GAD-7 Total Score" or "PHQ-9 Total Score".
Group — Choose the Outcome Measure Group that the measure belongs to. Groups help organise Outcome Measures and make them easier to locate.
Description — Optionally enter additional information about the Outcome Measure, such as what it is used for or any guidance for clinicians.
Unit — Specify the unit that the recorded value represents, where applicable. For example, you may use Points or another relevant unit.
Minimum value — Enter the lowest valid value that can be recorded for this Outcome Measure.
Maximum value — Enter the highest valid value that can be recorded for this Outcome Measure. These limits help define the expected scoring range for the Outcome Measure.
- Save the new Outcome Measure.
Edit an Outcome Measure
If you need to update an existing Outcome Measure field:
- Go to Main Menu → Fields → Outcome Measures.
- Select the group the Outcome Measure belongs to from the Group dropdown
- Click the Pencil icon next to the Outcome Measure

- Update the required information.
- Save your changes.
Make an Outcome Measure Inactive
If an Outcome Measure field is no longer required, you can make it inactive.
- Go to Main Menu → Fields → Outcome Measures.
- Select the group the Outcome Measure belongs to from the Group dropdown
- Click the Pencil icon next to the Outcome Measure

- Untick the Active checkbox
- Save your changes.
Inactive Outcome Measures are no longer available for new use and any previously recorded values will be removed from reporting.
To view inactive Outcome Measures, tick the View Archived checkbox at the bottom of the Outcome Measures table. To make an Outcome Measure field active again, edit it, tick the Active checkbox, and save your changes.
Merge Outcome Measures
If duplicate Outcome Measure fields have been created, or you want to consolidate data into a single Outcome Measure, you can merge them.
- Go to Main Menu → Fields → Outcome Measures.
- From the Group dropdown, select the group the Outcome Measure you want to merge belongs to
- Click the Merge icon next to the Outcome Measure

- Select the Outcome Measure that you want to merge it into from the dropdown

- Click Merge
Merging combines the recorded data from one Outcome Measure into another, helping keep reporting accurate and avoiding duplicate measures.
Important: Before merging Outcome Measures, ensure you have selected the correct destination measure. Merging may affect historical reporting and should be performed carefully.
