# View and Manage Patient Outcome Measures

Once [Outcome Measures have been recorded for a patient](https://knowledge.writeupp.com/en/record-outcome-measures-in-an-assessment?hsLang=en), you can view their results over time from the patient's record. You can also manually add historical values where required.

### **View a patient's Outcome Measures**

1. Open the patient's record using the Active Client button.
2. Select the **Outcome Measures** tab.

From here, you can review all recorded Outcome Measure values for the patient.

#### **Switch between Chart and Table views**

Outcome Measures can be viewed in two formats:

- **Chart** - Displays the patient's scores over time in a graph, making it easy to identify trends and progress.

- **Table** - Displays each recorded value in a tabular format, including the date and score.

Switch between the two views using the **Chart** and **Table** options.

#### **Filter the results**

Use the filters at the top of the page to find specific Outcome Measure data.

**Date filter**  
Limit the results to a specific date range.

**Group filter**  
Display Outcome Measures from a particular group, such as Mental Health or Musculoskeletal.

**Outcome Measure filter**  
View results for a specific Outcome Measure, such as GAD-7 or PHQ-9.

Filters can be used individually or combined to narrow the displayed results.

#### **Export Outcome Measure data**

To export the displayed results:

1. Apply any required filters.
2. Select **Export CSV**.

A CSV file containing the current results table will be downloaded.

### **Add Outcome Measure values manually**

In some situations, you may want to record an Outcome Measure without completing an assessment.

For example, you may wish to:

- Record scores from historic paper assessments.
- Record scores from another clinical system.
- Backfill older patient data.

To add a value manually:

1. Open **Patient Summary → Outcome Measures tab**.
2. Select **Add Value**.

3. Choose the required Outcome Measure.

4. Enter the value/score.
5. Enter the date the score was recorded.
6. Save your changes.

The manually entered value will be added to the patient's Outcome Measure history and will be included in charts, tables and reporting alongside values recorded through assessments.

### Outcome Measures Effectiveness Report

The **Outcome Measures Effectiveness** report provides an overview of the average monthly outcome measure scores for each clinician, helping you monitor progress over time.

The report includes:

- Average outcome measure value for each clinician by month.
- Aline chart showing how average outcome measure values for patients assigned to each clinician change over time, making it easy to identify trends and track improvements or deterioration in patient outcomes.
- Filtering options to view data for specific time periods, clinicians and outcome measures (where applicable).

This report is available from **Main Menu → Reports → Performance → Outcome Measures Effectiveness** and can be used to monitor clinical effectiveness, review trends and support service evaluation.
